A couple things. If you have a case on the phone, make sure it is not loose. I had a case that until I made sure it was on correctly, I could not get a triple click of the home button to work.
Also, the clicks need to be within about a half second of each other. Make sure in Settings, General Accessibility that the Accessibility Shortcut says VoiceOver.
