The Taxonomy codes are reported in PRV segments.
 
For the 837I (with Addenda), the only PRV segments that remain are for the Billing Provider (Loop 2000A), and the Attending Provider at Claim Level (Loop 2310A).  Taxonomy is not required on any other type of Provider.  In these 2 positions, it is "Required if it is known to affect adjudication".
 
On the 837P, on the other hand, Taxonomy can be reported via a PRV segment for Billing, Referring, and Rendering Providers at both claim and line item levels if it is known to affect adjudication.
