Dan's is correct. You need to
set the colors to 256. I had the same problem on with 8.0/2003.
Also, if you connect to the server via RDP you need to make sure that that you
set the colors to 256 for the RDP connection as well. Otherwise, while you
are connected anyone browsing the webpage would see black icons.

worked for me on this issue, was to always connect (if using remote desktop
Connection) to the server with color depth of 256 Colors.
The issue resolves,
once you connect and disconnect from the server. If you are at the system
console, run it in 256 color mode also. My issue reoccurs when
I connect to the server in High (16) or (24) bit, then goes away when I connect
in 256 colors. Hope this helps.
