Repeat By:

Install Mandrake 8.2 (works fine).
Upgrade RPMS using Mandrake 9.0 CDs (to XFree86 4.2.1 - but nothing else, other than 
glibc and direct dependencies. NOT kernel, etc)
Download current/latest NVIDIA drivers from NVIDIA (Build 4190, released December 2002)
Install as per NVIDIA's detailed instructions.
Configure your XFree86Config-4 correctly (or perhaps without any modelines at all?)
Start X, notice the screen resolution is < 1400x1050, and that you are permanently 
stuck in 256 colours.
Read log file - notice that you allegedly have a "1400x1050 32-bit screen, which is 
only capable of 1024x768 in 256 colours".
