Well, yes, it's probably good to get to know people better but perhaps
the means by which you do it is important too.  Is a written "survey"
the best way to do this?  To me it sounds like an overly formal,
structured, and (in my opinion) somewhat intimidating way of going
about it.

Imagine someone coming up to you & saying "Hey, I don't know you but I
want to get to know you better!  Here: this is an opportunity to
express yourself!  Just fill out this form!"...

as opposed to:

"Hi, I see you're new here... welcome."
