You seem to forget that ABC players also should be able to make
sense of the notation.

I suggest the following:
1) [K:D exp _b _e ^f] will accept only lowercase accidentals
that apply in all octaves.
2) [K:D oct _B,,, _e'' ^F] will accept octave sensitive
key signature definitions.

Only (1) will be adopted in the standard.

Programs that have need for octave sensitive key sigs
may implement (2) as a private extension.
