modprobe ivtv (seems to success, the attached log) The msgs generated by tveeprom do not appear to be correct.  Are you sure the correct tveeprom is getting loaded?  Other than that, I'd need to see your stuff from modprobe.conf to figure out what's going on.  And I'm guessing the ivtv list would be a better place for this question.
