Does dmesg show any ivtv errors when this happens?

No, nothing special. I've turned on debug messages also (ivtv_debug=203), but there was no scaring or at least unusual info logged either.

If you use the defaults for stream_type, bitrate and dnr_mode etc., do the
same problems occur?

I'll try it out. Jftr: my ivtvctl settings before 0.3.8 were:

opt | param
-u | 0xff (i.e. PAL)
-f | width=720,height=576
-c | aspect=2,audio=0xE9,bframes=3,bitrate_mode=0,bitrate=8000000,
    | bitrate_peak=16000000,framerate=1,framespergop=12,stream_type=10
-y | brightness=128,hue=0,saturation=64,contrast=64,volume=60000,
    | mute=0

With 0.3.8 i have added "dnr_mode=3" to -c because it seemed that noise filtering was turned on by default now, which gives smaller files, but also leads to slightly lower quality, esp. lower definition. Btw: it is wise to turn noise filters on by default, it is just no good for me because signal quality is excellent here and disk space is no problem.

Would you be willing to try to determine when this problem was introduced? It
could be time consuming but it may be the only way to find out where it went
wrong.

I can try to find this point, but the machine with the two pvr 250 is busy most of the time, so it may take indeed some weeks until i find something, because i can only do exessive testing during the rare idle times... and of course i have to watch at least two hours of recording time to be sure that the problem really is absent :-)
