I've met the same problem... installed, set login/pass, restarted... there was a setting menu but no "lasFM" menu.
I restarted, nothing changed. Switched to fishbone... and it appeared. Switched back to default... nothing... refreshed... there it was in default skin. Now it's in both skins, there is no problem. It could be both a problem with the cache (browser, SC...) or with my eye (didn't really know where to find the menu so I could have missed it). Anyway, no it's on and it's terrible!
