Eventually I found it comes from the document properties dialog 
(HeadersConfigurationPage.cpp).
Each time the dialog is opened the strings are read from the metadata then 
displayed in the dialog.
Each time the dialog is closed, the strings are converted to utf8 before  
being written to the metadata.
The conversion process is asymetrical.

Adding a conversion from utf8 when strings are read from metadata seems to fix 
the problem.

I just hope it's working for everybody...
