Here's a constructed reference example of the kind of phenomenon I was looking 
for.  This pattern occurs with considerable frequency in lots of copyrighted 
stuff I have.

You start with a leading "bar 0" with an 8th note in it, bar 1 to n are part 
of a repeating section leading straight into another repeating section, where 
the :||: comes physically somewhere on the inside of bar n.  On the far side 
of the |: (still bar n) there is another 8th note, the | to close out bar n, 
and so on in that fashion.

The final bar is usually short by the 8th note that was shifted to the left at 
the very beginning, but I have tacked on a final short 8th duration segment to 
fill out the final bar, so as to focus the problem away from a different 
problem involving an intentionally short final bar.

The 8th note in this example can of course be some other duration, but 8th 
notes are extremely common whenever I encounter this pattern.

None of the three new options can make any sense of this.  They all screw it 
up in different ways.  The new default option comes the closest to getting it 
right, but there are extra, empty, partial bars in the middle of it that 
shouldn't be there.
