Ex2013 running on 2012, this is the 2nd time in one week that email is down ,
it wasn't actually down, as activesync and MXalerts reported no outage. Once I
realized it was Outlook i went to check RPC service and sure enough it was
stopped.

On the W2012 server Ex2013 on-premise single server, the CU6 update removed
the SSL certs for mail.domain.com and replaced them with MICROSOFT EXCHANGE
certificates
just thought I'd throw it out there.

The ip not listed on ANY rbl, however rdns shows x-x-fios.verizon.net.
(that's the only possible reason I can think of getting bounced)

I had to pull UR8 v2 as it broke OWA -
Basic symptom
OWA page would not load under FF IE or Opera
funny thing was that on Android you would see a partial load (No images but you
could see credentials boxes, public private computer options, use the light
version)
I did try a reboot
you describe is typical behavior if setup could not copy the OWA files to
the proper location due to permissions.
