Do you know the make and model of your motherboard? Most boards with a 775 socket should have those 4 holes into which the 4 legs of the heatsink/fan fit. They usually use push-pins (not screws), and they should just snap into the holes without too much force.
If you know the make/model of your mobo you can find out what type heatsink it supports. But the main thing is, YES, I would definitely think only having 3 screws would cause the heatsink to not cool the CPU. Those 2 metal surfaces have to be perfectly flat and tight against each other for the paste to do its job. Having only 3 pins would raise one corner of the fan off the CPU surface - and probably cause overheating.
