Scott, In the heading of our policies at ODU, we list the "Date of Current Revision or Creation," which is the effective date of the policy. Our policies also include a signature page at the end, listing all of the approvals and dates through the review process, ending with the President's signature, which matches the date on the front page. Here is an example:
