We are looking to revise our review process, but our current process is here:

Most of our policies are on a four-year review schedule, a few are reviewed 
annually.  We do not have a specific office to address policy gaps, since the 
responsible official (usually a VP) over each area of operations is responsible 
for development of policies needed for their area of responsibility.  That 
said, in practice the General Counsel’s office often nominates policies for 
creation based on developing risks  or regulations.

Hello! I’ve just joined the group on behalf of Gonzaga University and I’m 
looking forward to collective insight on so many aspects of policy!

Initially, I would love to have samples of a review process and a schedule for 
existing policies. A second question for the group would be if you have a 
specific entity on campus that is tasked with identifying policy gaps (either 
vulnerability or cultural).
