Hi,

Not sure about the first two answers, but as for the black video, even though 
Premiere refers to it as black video, it actually makes a black still image.  
You go to FILE > NEW > BLACK VIDEO.  It creates a clip in your project.  Then 
you can just drag it onto your timeline and drag the end out to size it to 
whatever time you like, just like you would do with a still photo.  I use 
Premiere Pro 1.5, so maybe it's a little different.
