I think I’ve only had 2 licensed radios zapped (same event), and when I had them repaired, it was the power supply section that took the damage. So even with optical data and DC power, I’d say a lightning protector on the power input at the radio is recommended. The good news is there are less constraints building a good DC power protector because you don’t have to worry about affecting the Ethernet data. Just a gas tube and some husky MOVs or something along those lines. Probably the only good thing from my Purewave adventure is they told me they had basestations getting fried by lightning until they started telling customers to install a DC protector and that solved the issue.
