This is the boilerplate we've used. A bit less "legal", but gets the point across. It also asks for confirmation that they got it.

   Hello xxxx,

   Please review the claim of copyright infringement registered against
   your IP address, which is in conflict with our service agreement.

   The claim is that someone at your location is sharing copyrighted
   material with BitTorrent (or a BitTorrent-like tool).The details of
   the infringement are included in the message.Please note that the
   timestamp is "Z" or Zulu, which is Greenwich Mean Time
   (GMT).Subtract 8 hours to get local time (7 during Daylight Saving
   Time).

   We must instruct you to cease this activity immediately and take
   action to prevent it's future occurrence.We take this very seriously
   as it can affect our ability to legally continue providing service
   to you, as well as representing a threat to our entire business.

   Please reply to confirm you have received this notice. As well, let
   us know if you have any questions. Our experience with this issue is
   that usually our customers in this situation are unaware that this
   activity is being done by visitors or family members using their
   service.
