Can't you buy the needed parts and configure the PCB the way you want and do it all yourself? Like with a built-in RJ45 input?

Here's one that's a bit smaller, but probably still too big. who knows if it will be around for awhile... it's direct from China, but there's a ton of this sort of stuff on ebay and amazon.

        We have used these to power radios off batteries for
        installers. They are adjustable - we set them 48v out on

        Looking for the smallest cheapest 12 volt to 48 volt unit
        that will do 20 watts.

        Digi-Key has some but they are not cheap. Looking for
        something in the $20 range.
