I would go with moving the ears back.
You may have an issue with internal clearance, but it shouldn't be that bad.

I've seen lots of M6 in the stuff from China, and of course 12-24
and 10-32 from US sources... But never M5.

the holes would line up so you’d get more switch in front of
the rails.

This is really specific; don't ask why I need it :)
But I need a standard rack threaded screw that looks like
a 6 to 7 inch long motherboard standoff screw.
So it would stand a 1U switch forward from the 19" rack
about 6-7 inches.
I tried standard rack standoff modules, but the space is
too limiting.
