I have similar problems with my hostap driven card (but it succeeds on
2nd try in most cases).

I have tried to debug it, and it seems, that if wpa starts to check for
networks too early after card firing it up, card does not work properly
and does not repeat any results in networks scan.

I don't yet know, what is the source of this problem, but adding sleep
command somewhere in /etc/network/if-pre-up.d/ may work-around it.
