OK I tried that. Yes there are plenty of records in the form. I already
had escalation logging turned on and it states Failed Qualification.  In
the SQL log I can see the select statement. No errors. 

If the escalation log says failed qualification I assume this is
referring to the Run If statement. Is that correct.
