I believe with 6.3 there was a patch level that had bugs with escalations. You'd get "ghost" escalation threads, and you could only see them by looking at weirdness in the logs. What I did was to disable escalations (server config), which would shut down all the escalation threads. Give it half a minute to "take", and then re-enable escalations. Its still not warm and fuzzy, but it keeps you from having to experience the cold emptiness of a server restart. Metaphorically speaking.
I don't remember which patch fixed it, but it did get fixed at some point.
