The best way to do custom email is to use HTML templates. You create an HTML page with your desired page layout and include Remedy form fields by encapsulating the field name like so: #$$Summary$$# then create a new entry in the Email Templates form and attach the HTML file.
When creating or modifying notifications you can configure it to use this template. Be sure to include in the filter all fields defined in your template. If you don't, the resulting email will show the field name text (like indicated above) instead of the field value.
