On the Miscellaneous tab of the Company form (at least in 8.1) there are two fields that you could use: Hot Line and Email. You'd still have to do the look ups when sending out the email, but at least the data would be in a sensible place and you wouldn't have to create a new form. You could use the Notes field (or re-purpose one of the other ones) to store the template name.
