I can attest to this. I have been with them for almost 2 relatively
(aside from a few outages) problem-free years. Unfortunately, their
support has gone from veeerrrrryyyyyy sloooooowwwww to virtually
nonexistent. My last payment didn't even show up in my account, so I
decided to cut my $20 loss and leave them.
I believe AtlasVoice can pass both callerid number and name - you might
want to check them out. If you don't mind a US provider, I think VoipJet
does too (oops, scratch that - I just tried it and only the number
showed up)

Link2voip works for that. Shows the number but not the name.
