When I did my rebuild I had to completely recreate my partitions since
the recovery partion had been wiped out on the first hack I did. I
then used Scott's manual recoveryethod that you can find on this site
(I would give you a link but I am working from my iPhone while at
work) to setup the Recovery partition with version 1 followed by a
factory reset.

I doubt that it is a flash memory failure as the memory needed vs what
is installed usually allows for sector failures.  I also doubt that it
is a power issue as there really isn't much power required for this
type of opperation (far less than running the hard drive or USB
stick.  I would be more prone to getting the ATV v1 image which can
still be found on the net but is in the other units you own and
rebuilding using Scott's instructions. I would not do a DD recovery
from the other units just to be safe for now.

The fact that you are able to recover to V1 with remote support is
what makes all of this odd.  Perhaps try a restore of v1 and then do a
couple more restores of v1 just using the apple interface. This would
be to see if the initial restore didn't completely work.
