[authors] Impress Guide nearly finished

I have now completed the indexing of the Impress Guide. None of 
the chapters are checked out for review, so it appears that no 
one is doing any reviewing. Therefore, I am putting together the 
full book with a view to publishing the first edition later this 
week. I don't see any reason to hold it up any longer.

As always, published chapters can still be reviewed (and I'm sure 
the index needs improvment; my indexes always do) and the book 
will be updated as changes are made. Also -- Peter, I know you 
are working on a set of replacement screenshots; when you have 
them done, then I will update the published chapters.

Thanks to everyone who has helped with this book. We are getting 
close to having a full set of books completed (except for the 
Base Guide), and I think this is a very exciting prospect.
