The construct of the old user management on the old site was the result
of the ugly and non working work flow on the old instance.
We do not needs more than for rules:
Manager - The ones who are able to manage site and users
Contributors - The ones who write or translate documentations
Reviewer - The ones who review documents for the later editing
Editors - The ones who are able to publish the final documents.
For the English section, I do not see any reason to have a difference
between contributors and reviewers; everyone should be able to do all
the same things. But if that is better for the other sections, then I
will just assign both roles to all our people.
All tasks can be built on this rules, because You can assign to or more
rules to a person ;-)

Ans Yes its a fault, that visitors without member right could not see
the site content. That's why only external published content will be
visible for the public.
I think it was that way on the old site, too.
We have to set this state for the needed pages as Andreas said it...
I have been publishing the sections and pages in English that the public
should see, but I have not done the other sections.
On another matter: the English section needs 4 levels of navigation
visible on the left-hand side. If this is too many for the other
language sections, is there some way to have different levels for
different parts of the site? It is very annoying to try to navigate the
English section with only 2 or 3 levels visible.
