There were 2 sets of papers.  The first papers (Declaration of Intent) and
the 2nd papers (Petition for Citizenship).  Even though they are very
similar, ask for ANY AND ALL paperwork.  Depending on the rules of that
time, they can be filed a few years apart.  Something they remembered at
one time they may have forgotten by the next time.

The certificate of citizenship number is different.  If you scan the form
and send it to me I can tell you what numbers to use.

You may or may not need the index search first.  I'll have to look on that
certificate for any other numbers.
