Petition number is 288261. Number 4214379 is just the certificate number. I'm 99% sure that I used the petition number, but those G-1041 forms are quite different than the old form. They want a C number and now that looks like the 4214379 certificate number. You might want to enclose a copy of that naturalization certificate with your request.
