Yup, I can confirm if there is no Cache folder, then it will not start.
I renamed mine and ran squeezesvr.exe from a command prompt:
C:\Program Files (x86)\Squeezebox\server>squeezesvr
DBI connect('dbname=C:\ProgramData\Squeezebox\Cache\cache.db','',...)
failed: unable to open database file at /<C:\Program Files
(x86)\Squeezebox\server\SqueezeSvr.exe>Slim/Utils/DbCache.pm line 213
Delete the file C:\ProgramData\Squeezebox\Cache\cache.db and start from
scratch.
Renamed the folder back and it started OK.
I thought 'Run Cleanup' only cleaned the contents of the Cache folder,
but would not actually delete it ??
