It sounds a little like a hardware fault but clutching at straws do you have the RealTek sound card? This has a horrible plug an play feature which has unpredictable accessibility . Basically the driver for this device relies on putting up a window for you to confirm what device you have plugged in to the PC. This device will not work until you have confirmed this. If you have any sight at all this will flash up as a window when you plug your headphones in. Your screen reader should be announcing something even if navigating through this window is difficult.
Apart from that I can only suggest you update your sound card driver to eliminate any driver issues. If this does not work this does tend to point to a hardware fault. I checked my sound properties and the line out on my PC is listed as one device even though like you I can access this from the front or the back so the potential for configuration seems very limited.
