OK, I've built various sets of docs: and they all show the new style headers 
and footers now.

However, if I build the doc_test\grouped_links\example\boostbook_integration 
example, then I don't see the new-style drop down lists or navigation 
controls, even after adding the missing stylesheets and images.

BTW, I'm using a fairly recent Docbook cvs "snapshot", but I can easily 
switch to an earlier stable release if that helps.
