Matias, this looks fantastic!

I tried to apply the new styles to some existing docs: basically just copied 
the xsl to the appropriate Boost directory and rebuilt, but then I see no 
headers and footers at all.  Same thing if I try rebuilding the test 
document.  My guess is you didn't commit the right version of the xsl stuff 
(or missed something?).
