I've definitely heard it used to described blacks - mostly by other blacks. As in when grandma might say "Boy, get your nappy head over here and do your homework". It also refers to diapers and, as I've learned, small, multi-use bowls (as for candy). Learn something new every day! But I can't find a dictionary that refers to it as a "slur". Google it - on the first page are at least six or eight groups who apparently don't know that it's an insult. They're using it to describe support groups, inspirational groups, clothing lines, hair care support and many other things.
