Our cat have very similar problems as you indicated.
We used Frontline spray from petcarerx.com
We also turned and bombed the house to kill any potential
eggs that may still be around.

Now for the cat, the cat was happier and did not have
many sores remaining and we noticed he was scratching
a lot less than he use to. We plan on doing another
treatment soon on him to try and make sure we caught
everything.
