I hand code everything.  And I'm not trying to sound snobby when I say 
that, really, I just have never found a good enough tool that creates 
easily maintainable, clean html code.  They all add in shims that are 
nearly impossible to deal with maintenance-wise, unrecognizable image 
names, the list goes on.  Most code generators also create quite 
un-search engine friendly code, which is a huge part of our business.  I 
can't tell you how many sites we've basically rewritten that were 
created with CS, DW, and ::shudder:: FrontPage.

Also, we're creating more and more CSS-based layouts, which, as far as I 
know isn't something that code generators have figured out how to do yet.
