It is a spay, but the organs are so bloated and full of infection that
it is much more risky and takes longer.  They have to be very careful
to contain all the infection of the dog could become septic and die
from that.  I don't know why they charge SOO much more, but they do. 
They also give some medication (and IV drip- which I know is expensive
by itself) after the spay, but I'm not sure how much that factors in.
