You may ask your vet about giving oral ivermectin.  

Demodex, as I'm sure your vet explained, occurs normally on ALL dogs -
 no exceptions.  It's only a problem when the immune system becomes 
compromised for whatever reason.  Then, the body can't keep them 
under control.  Dips and ointments can work over time.  Sometimes, 
though, the mites are deep in the skin and an oral solution is the 
key to getting it under control.

Since your regular vet has not, from what you've said, offered this 
soluation (or really any other) I would see a veterinary 
dermatologist.  After all, this is a skin condition and that's what 
they specialize in.  After my  one dog started developing ear 
infections and skin infections that didn't respond to normal 
treatments, my vet recommended a dermatologist.  Just like human 
doctors, they specialize for a reason.  Not every doctor can know 
everything about everything.  Anyway, it was determined by the 
dermatologist it was food allergies.  Changed food and treated with 
oral ivermectin.  Done.  Cleared up quickly and has yet to return.
