Sounds like you are doing everything right.  I am stumped and have no
answers.  In 26 years I have never had any problems housebreaking normal
sized Cavaliers--they are literally all 95% there by 12 weeks of age.  But I
have had some real problems with the little ones--those that are under 2 1/2
lbs. at 8 weeks and under 5 lbs. at 12 weeks.  I think about now I'd be
resorting to a larger crate with a wire grate to prevent the dog getting
dirtied--until she catches on.  I wouldn't have the time to clean up and
wash a puppy 5 or 6 times a day!

The Innova can be a problem in some Cavaliers.  You might want to change
that for the time being.  I had 2 girls that just could not eat Innova
without excess pooping.  Had much better luck with Canidae.
