Put a leash on your puppy and hook it to your belt so that she must ALWAYS be with you when in the house. You will be able to see every move she makes and can take immediate action. If she is not attached to you, she is in a small crate. She is never to have free run of the house at any time. Feed on a strict schedule so you will be more aware of when she may have to go out. Also, remember that she is just 14 weeks old toy dog and they truly can't go very long and may very well not be ready at all!! I have two kids who were years apart in being pottytrained. Dogs are no different.
I do understand the frustration though. I had a Cavalier that would go outside in the morning and run around like mad so she wouldn't have to "pee" outside. As soon as she came in the house she would go potty. It's almost like she was afraid to go outside and much preferred to go inside. I tried everything with her. She wouldn't go if she was "attached" to me but instead she would panic at the lack of freedom. She was 3 years old when she came here and I wonder what her life was like before I got her. I also believe that there are dogs out there who will never be housetrained no matter what you do. We all have the answers, but sometimes they just won't be housetrained.
