We've seen this only twice in 21 years of breeding Cavaliers and it has always responded to antibiotics (gentacin) without surgery. Now, in a Cocker, surgery was necessary, but not in a Cavalier.
I'd try Gentak first or the triple antibiotic eye ointment and save the surgery for if the medication doesn't cure it. There are serious secondary problems with the surgery so it should only be as a last resort.
