I had the same scenerio with an annual that was due while the resident was in the hospital. Upon return, we completed a sig. change assess. She was re-admitted to the hospital prior to careplanning. I was in a dilemma as to what to do with the assessment so I called up the New Jersey MDS lady. She told me I should send the assessment but to put "NO" to all areas that triggered on raps since we did not due any c/p. When she would return, we can do the raps at that time. ' Hope this helps
