Which "cert" are you talking about??   We take an eval and treat order on
admission.  We then do a telephone order with clarification ie., PT 5X/week
for 4 weeks for specific modialities.  We send out the HCFA 900 ( The
therapists eval and  cert form) for the doctor to sign.  That has alot more
specifics.    We then also send out a Brigg's form 888 Certification and
Recertification form.  I write the same on of them "PT 5X/week X4weeks for
ROM, transfer and gait traning.  OT 5x/week for balance and ADL's. " and
then ST if appropriate.  Do we need to be doing all this?  And am I doing
this Cert/Recert form ok?"

long as I can remember and I've been in LTC for a LONG time.
