A RAP can be many different things. It does not have to be a narrative note, checklist, etc if you can find your information in nurses' notes, MD progress notes, etc. to cover the needed material in a rap summary. Of course, this is very hard to do and narrative notes, checklists, etc. are usually the easiest way to fly. See pg. 4-11 - 4-15 of the MDS manual.
