I think we need to find out what the DON means
when she says she doesn't want the RAPs "done." If she is referring to
writing a RAP note or some other form of summary or checklist relative to the
RAPs, then she is correct - there is no regulatory requirement to do these
things, as long as the information listed at the top of Section V on the MDS
can be found somewhere in the chart. That information must appear in the
Location and Date of RAP Documentation column of Section V.
I don't
recommend not doing a summary that provides all of the information listed at
the top of Section V - but there certainly are facilities that choose to do it
that way.

WHAT???????? Your DON does not want to do RAPs?
Do you mean she doesn't want you to do them? It's not
optional. The RAPs are part of the RAI process and are required for
comprehensive assessments.
See page 1-8 of the RAI
Manual:
"All comprehensive RAIs authorized by states include at least
the Centers for Medicare and Medicaid Services' (CMS's): *MDS Version
2.0 (with or without optional sections S, T, U) *Resident Assessment
Protocols (RAPs), including -Triggers -Trigger legend -RAP Summary
Sheet"
I don't believe there are any regulations as to the format of
the RAPs, but there is no basis for not doing them.
