Just an addendum. When we first started with RAC and we did the entire MDS and Raps, 
we expected the other disciplines to complete their assessments during the 7 days 
prior to the ARD so that we could glean from them to complete the MDS process. After 
they again began with their own sections of the MDS, we had them do the paperwork 
after the ARD as a reflection of the tracking period while merging the data for their 
RAP. (Hope this makes sense)

Funny this should come up. We also have had some differences of opinion and I am 
looking for clarification.
The Resident Assessment Coordinators complete the MDS and RAPs on behalf of the 
nursing dept. The other depts. are responsible for certain sections of the MDS and 
related raps. The Nursing dept completes Continence, Safety, Hydration, Skin, Sleep, 
Wandering, Eating, and Pain Assessments on a quarterly basis aligned with the MDS.

As far as timing:
In order for me to glean the information related to Nursing assessments, they need to 
be completed during the 7 days prior to and including the ARD. ... Correct?

For completion of MDS and Raps and related other discipline assessments, they should 
reflect data that occurred during the designated time periods ie. 7 d, 14 d, 30 d, but 
be actually completed after the ARD (up to the date the assessment must be closed and 
locked).... Correct?

Please share your views. We currently have 4 RACs and we didn't know until recently 
that we had varied opinions.

I think that we are getting tripped up by the word assessment.  You would
assess the skin, vision, hearing, vision, etc DURING the observation window.
However, if you are talking about having a dated note from each of the
disciplines to be able to use to enter data for the MDS, I think that their
note should be AFTER the ARD and should reference the 7 day observation
period.  If the ARD was on Monday the 9th, the dietitian could enter a note
into the record on the 10th stating that the information for section K was
for the observation period of the 3rd to the 9th.  This should satisfy the
surveyors--that you are all looking at the same observation window and the
accurate date of entering the data into the medical record is present.
