Donna, I think that we are getting tripped up by the word assessment. You would assess the skin, vision, hearing, vision, etc DURING the observation window. However, if you are talking about having a dated note from each of the disciplines to be able to use to enter data for the MDS, I think that their note should be AFTER the ARD and should reference the 7 day observation period. If the ARD was on Monday the 9th, the dietitian could enter a note into the record on the 10th stating that the information for section K was for the observation period of the 3rd to the 9th.
