I send a form out 2 to 3 times a week, depending on how many admissions.
The form has name, ARD and the date 5 days after the ARD. The staff has to
have their info in the computer or on my desk by day 5 so I can finish up
the MDS. I started this back when the state of IL. said that all MDS's had
to be submitted to the state within 7 seven days from R2b. Luckily that
didn't last long but the staff liked the form so I continue to use it.
